Registration Packets for the 2023-24 school year are listed below, they are also being sent home with your child. Registration packets must be filled out every year by both new and returning students. As always, we ask that you carefully read through each page and complete all forms that are necessary for your child. All registration forms should be completed and returned to our office no later than Tuesday, August 1, 2023. If you would like to get a jump start on registration you may begin dropping off, mailing or e-mailing your packets starting April 24, 2023. If you would prefer to register In-Person, the registration date will be Tuesday, August 1st from Noon - 6:00pm. If you have any questions please feel free to call the school office at 815-627-2671.

If you are transferring in from another school district:

If you are transferring from another district it is required to have the following in addition to the regular student registration:

• Request for Student Records Form

• Completed Registration Packet & Residency Requirements

• Up-to-Date Immunizations, Health Exam, Dental Exam and Vision Exam

If you are new to Illinois:

If you are new to Illinois it is required to have the following forms in addition to the regular student registration:

In-Person Registration:

In person registration will be on Tuesday, August 1st from Noon-6:00pm.
