More Registration/Health Form Information

From the Nurse: We will need the following forms

• Kindergarten students:  school physical, eye doctor form, dental form*
• 2nd grade students - dental form*
• 6th grade:  school physical, dental form*
• 9th grade: school physical*
• Sports physicals for all students who are going out for sports at Paw Paw Jr./Sr. High School. 

*All  students are eligible to take advantage of the mobile dentist to be held in October to fulfill the requirements for a dental exam for K, 2nd, and 6th grade or you may go to your family dentist.

Health Forms:

• Physical -- required prior to Kindergarten, Sixth, and Ninth Grades
• Sports Physical -- required yearly to participate in athletics
• Vision Screening -- required prior to Kindergarten
• Dental Screening -- required prior to Kindergarten, Second, and Sixth Grades
• Authorization for Administration of Medication Form -- required prior to the administration of any medicine, including over the counter medicine such as aspirin, to a student by the CUSD 271 staff.
